1/4" output jack and one high quality 18mm Piezo pickup
These are the same parts I use on my own instruments.
The jacks are longer than average and accomodate the thickness of most cigar boxes.  When mounting the piezo under a resonator cone I recommend using 3M double sided adhesive squares. I do not let the disc touch the cone. Experiment with different placements of the Piezo. I find that I get a better overall tone from the instrument by placing the piezo next to the cone under the lid. I prefer the deeper tones from the low string side of the cone.
